Procedure for Obtaining a School Transcript

The “Family Educational Rights and Privacy Act of 1974” prohibits the dissemination of information from student records without the written consent of the student if he/she is eighteen years of age or older, or the written consent of his/her parent(s) or legal guardian if he/she is under eighteen years of age.

The procedure for requesting a high school transcript or copy of a student’s record is listed below for your convenience.  Please note that the procedure for requesting a student’s immunization record is the same. Copies of student records, transcripts, or immunization records are provided at no charge and require 5 Business days for processing.  They may be picked up on Mondays and Thursdays between the hours of 8:30 and 3:30.   Please be aware there is only one Records Clerk in the county, therefore, due to vacation or illness, the procedure of five days may need to be extended when she is out of the office.

  1. Submit a written request –

  2. Include your FULL name when attending school
    1. Please indicate the last name used on school records (for example, we require the maiden name of women who have married since leaving school or the married name for women who married while attending school)

  3. Include the last Chesterfield County School attended

  4. Include your graduation date or the last year attended

  5. Include whether the transcript should be official or unofficial

    Official-to be used for DMV, Education, Vital Statistics, Employment –-received in a sealed envelope NOT to be opened by student

    Unofficial-to be used for personal records or employment – received in envelope accessible by student

  6. Include the last four digits of  your social security number for verification

  7. Include your date of birth

  8. Include your daytime phone number
    – In the event additional information is needed

  9. Include the complete mailing address or fax number for sending the transcript.  Please note: A contact name must be included with a fax number

  10. In the event you want another individual to pick up the transcript, you must indicate this in your request and include the individual’s name

  11. Sign your request

  12. Mail transcript requests to:

or fax your request to the office of Student Records at (804)279- 7320, Attention: Darlene Blanks, Records Coordinator

You may also pick up your transcript at the Thomas R. Fulghum Center, 4003 Cogbill Road, Richmond, VA  23234, between the hours of 8:30 am and 3:30 pm on Mondays and Thursdays, five business days after your request is received.   We are located in the Meadowbrook Area of Chesterfield County.  Please be aware there is only one Records Clerk in the county, therefore, due to vacation or illness, the procedure of 3 days may be extended when she is out of the office.

From Chippenham Parkway; take the Hopkins Road exit and go east to Cogbill Road.  Turn right on Cogbill Road then right into the second driveway by the marquee for the Thomas Fulghum Center.  Continue through the parking lot to the front of the building and enter through the glass doors.  Upon entering the building you will find a table located to your left.  On this table you will find a Transcript Request Form to be completed, as well as the procedure for requesting your transcript.  Once completed, the Transcript Request Form, along with your identification, will be verified so that your transcript can be processed.
