The below forms are required after you have received written notification that your Substitute Application has been selected for consideration. 
(DO NOT SEND FORMS UNLESS CONTACTED BY HUMAN RESOURCES)

APPLICATION AND FORMS REQUIRED FOR SUBSTITUTE TEACHING AND/OR SUPPORT POSITIONS

Substitute teaching in the classroom REQUIRES one of the following to be sent with required forms:

  • College transcript showing total credit hours taken (minimum of 60 hours –
    they do not have to be official)
  • College transcript/diploma showing graduation date (they do not have to be official)
  • A letter from the college stating requirements have been completed for degree and date complete
  • If applicable, a statement of eligibility or copy of teaching license (expired or current)
